Prior to the expiration date noted on your VDH Permit to Operate a Foodservice Establishment you will need to renew your permit every year. At the end of each quarter, the Hampton & Peninsula Health Districts will mail or email an invoice to permit holders, advising them to renew their existing permits. To avoid operating without a valid permit, the annual permit must be renewed prior to the expiration date.

STEP 1:  Complete the Annual Permit Application

You will need to complete the Application for Food Service Permit and pay the $40 annual renewal fee.

STEP 2:  Submit your completed Annual Permit Application

• Scan it into your computer and email to your local health department where the facility is located or based. You can then follow up by calling the locality where your operation is based to pay the $40 annual permit fee with a credit card; or
