VitalChek can be reached through their <span style=\"text-decoration: underline\"><strong><a href=\"https:\/\/www.vitalchek.com\">website<\/a><\/strong><\/span> or toll free at 877-572-6333. An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express\u00ae, Discover\u00ae, MasterCard\u00ae or Visa\u00ae.<\/p>\n<p>The Virginia Health Department (VDH) has contracted the VitalChek Network to help people get a certified copy of a birth, death, marriage, or divorce certificates within 2 to 5 days! Once VDH has received the VitalChek information, VDH staff members will search, print, and send a certified copy of your request. <strong>Please note that you will be required to sign for the UPS package.<\/strong><\/p>\n<p><strong><u>Fees Charged:<\/u><\/strong><u><\/u><\/p>\n<ul>\n<li><strong>$20.80 \u2013 For each certified record<\/strong><\/li>\n<li><strong>$12.70 \u2013 VitalChek Service Fee <em>(This fee is non-refundable)<\/em><\/strong><\/li>\n<\/ul>\n<p><strong><u>Delivery Option Fee<\/u><\/strong><strong>:<\/strong><\/p>\n<ul>\n<li><strong>$19.50 \u2013 UPS Air \u2013 cannot deliver to a PO Box<\/strong><\/li>\n<li><strong>$18.00 \u2013 UPS 2 Day Air \u2013 cannot deliver to a PO Box<\/strong><\/li>\n<li><strong>$00.00 \u2013 United States Post Office Regular Mail (No Fee)<\/strong><\/li>\n<li><strong>ALL FEES ARE SUBJECT TO CHANGE<\/strong><\/li>\n<\/ul>\n<p>A condition for using this system is that payment must be made by credit card. You can phone in your request by calling 1-877-572-6333 and pressing option 1. To check on the status of your request through VitalChek, call the number above and press option 2. You may also order online at <a href=\"https:\/\/www.vitalchek.com\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>https:\/\/www.vitalchek.com<\/strong><\/a>.<\/p>\n<p>In addition to a credit-card number and expiration date, we require the following information:<\/p>\n<ul>\n<li>Type of certificate you are requesting<\/li>\n<li>Relationship to person named on the certificate<\/li>\n<li>Full name<\/li>\n<li>Date and place of event<\/li>\n<li>Mother&#8217;s full and maiden name<\/li>\n<li>Father&#8217;s name if requesting a Birth Certificate<\/li>\n<li>Full name of husband and wife if requesting a Marriage or Divorce Record<\/li>\n<li>Area code and daytime phone number<\/li>\n<li>Street Address, City\/County, State and Zip Code to send the certificate<\/li>\n<li>P.O.
