A year-long effort, the CHIP was developed through research, collaboration and strategic planning. The CHIP is an action-oriented document based on the findings of the Community Health Assessment (CHA). Released in April, the CHA examined a variety of data sources to understand key health needs. The community then selected the top three health needs, which are addressed in the CHIP.<\/strong><\/p>\n

The 2025 Community Health Assessment (CHA)<\/strong> data collection process is now complete! Thank you to all community members and partner organizations who participated and shared valuable insights. Your input is essential in understanding the health needs and strengths of our community. The Rappahannock Area Health District, in partnership with Mary Washington Healthcare, is now analyzing the data to identify key priorities. A draft of the findings is now available, guiding the development of our Community Health Improvement Plan (CHIP)<\/strong> to create meaningful, community-driven solutions.<\/h3>\n

What is a CHA\/CHIP?<\/span><\/span><\/h2>\n

A Community Health Assessment (CHA) is a detailed study of people’s health in a community. It looks at health problems, what resources are available, and what needs to be improved. This helps community leaders understand the most important health issues to focus on. A Community Health Improvement Plan (CHIP) is created based on the CHA. The CHIP is a strategic plan designed to address the community’s biggest health concerns by setting specific goals and actions, such as reducing smoking, promoting exercise, or improving access to healthcare. The CHIP involves people from different parts of the community, such as local governments and healthcare providers, working together to improve health. It\u2019s a long-term plan that\u2019s regularly updated to ensure progress and that the community’s needs are met.<\/p>\n
