CHD works to provide critical health education and promotion information on a wide variety of topics, including healthy behaviors, health risks, and emerging health threats.<\/p>\n<h3><strong>Preventing and Mitigating the Harm of Chronic Diseases<br \/>\n<\/strong><\/h3>\n<p>The CHD Population Health Program supports and leads multi-sector partnerships to prevent chronic diseases and limit their impacts by supporting policy, systems, and environmental changes that promote:<\/p>\n<ul>\n<li>Food security<\/li>\n<li>Healthy eating<\/li>\n<li>Active living\/active transportation<\/li>\n<li>Tobacco control\/cessation<\/li>\n<\/ul>\n<h3><b><strong>Promoting Trauma-Responsiveness<\/strong><\/b><\/h3>\n<p>Crater Health District convenes the Southside Trauma-Informed Community Network, a collective impact initiative supported by community members and public-private partners.
