You must have identification and proof of income when services are provided. The following is a list of acceptable proof of income to determine your Financial Eligibility for services:<\/p>\n<ul>\n<li><span style=\"color: #800000;font-family: Arial, Helvetica, Geneva, sans-serif, sans-serif\">Pay Stubs &#8211; Provide either the most recent THREE months or the most recent if &#8220;Year to date&#8221; income listed<\/span><\/li>\n<li><span style=\"color: #800000\">W-2 Form or income tax return for most current calendar year<\/span><\/li>\n<li><span style=\"color: #800000\">Employer&#8217;s written statement regarding weekly, monthly, or yearly income<\/span><\/li>\n<li><span style=\"color: #800000\">Unemployment notice from VEC (Virginia Employment Commission)<\/span><\/li>\n<li><span style=\"color: #800000\">Letter of Termination\/Layoff notice from previous employer<\/span><\/li>\n<li><span style=\"color: #800000\">Statement of Social Security benefits or other benefits<\/span><\/li>\n<li><span style=\"color: #800000\">Military LES (leave and earning statement)<\/span><\/li>\n<li><span style=\"color: #800000\">Foster care statement from Social Services<\/span><\/li>\n<li><span style=\"color: #800000\">Food stamps &#8211; Letter of eligibility<\/span><\/li>\n<li><span style=\"color: #800000\">TANIF check stub<\/span><\/li>\n<li><span style=\"color: #800000\">Medicaid card for current month<\/span><\/li>\n<li><span style=\"color: #800000\">Free or reduced school lunch &#8211; letter of eligibility<\/span><\/li>\n<li><span style=\"color: #800000\">Minors if not working must provide parents income to determine eligibility<\/span><\/li>\n<li><span style=\"color: #800000\">Must have proof of residency<\/span><\/li>\n<li><span style=\"color: #800000\">Must have proof of Identification<\/span><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>We offer a range of different services to the Chesapeake community.\u00a0 Costs for medical services range from free to full fee based on the service provided and you and your families financial eligibility.
